What is the importance of occupational therapy intervention?


Occupational therapy intervention comprises a set of person-oriented programmes. Their design helps to support the client with the performance of everyday tasks and activities. 


At our CHD Rehabilitation Services, we utilise this approach often to maximise the opportunities of those in our care.

Occupational Therapy Intervention - At CHD Living, we have an incredible team of OTs.

Supporting a Resident Using Occupational Therapy Intervention: 

At Kingston Rehabilitation Centre, we’re proud to have a compassionate and dedicated team of staff. Our Occupational Therapists offer specialist assessment and treatment with practical support. The ultimate goal is to maximise the functional independence of our clients. 


Recently, our Occupational Therapist & Therapy Team Lead, Leanne McHugh, helped transform a resident's life using occupational therapy intervention. 


The resident was a young person who had been using our rehabilitation services for 2 years. They needed long-term disability management and maintenance therapy following a spinal cord injury. 


Spinal cord injuries are complex as they cause permanent changes in motor function, sensation and other body functions below the point of the injury. The resident had a loss of function below the shoulders, affecting the use of their limbs. This had an impact on their ability to complete daily tasks such as self-care, mobility and accessing technology. 


In this case study, Leanne’s primary goal was to help the resident re-engage in education. To achieve this, they wanted to start a course at college, with the long-term aim of gaining employment thereafter.

How was Occupational Therapy Intervention conducted?

There were various areas to explore when supporting the resident back into education. Physical, social and emotional perspectives are all important when conducting the intervention. 


Firstly, Leanne encouraged the resident to explore courses of interest. Occupational therapists consider it important to work towards something meaningful and provide encouragement. This enables the resident to feel empowered, helping them progress towards their goals.  


Once the resident had selected a course, Leanne then facilitated a meeting. She convened with the college course leader, the college inclusive learning team and the resident. 


The team needed to identify the accessibility of the college from a wheelchair mobility perspective. They also had to determine how the resident could access the learning aspect of the course. 


It was important to consider and assess the course structure, taking into account the course materials, computer access, software programmes. They also had to see if there would be any presentations and group work that would need completing.

Occupational Therapy Intervention - Leanne McHugh transformed a resident

Occupational Therapy Intervention in Practice 

As the resident had no use of their upper limbs, the team determined that assistive technology was essential. This would enable the resident to access a computer for note and assignment writing. They established that the programmes and software used on the course would be compatible with assistive technology.


As it became clear that the resident could access the course, Leanne made recommendations to the college with the support of our neurorehabilitation consultant. 


Together, they constructed an educational healthcare plan to adhere to the resident’s physical and educational needs whilst at college. It included environmental adaptations, such as preferential seating and access to a wheelchair-accessible classroom, a support worker to attend college to support care needs, and ensuring the resident had the right technology to support learning. 


The resident also required access to the computer so they could engage with the course. Thus, Leanne facilitated an assistive technology assessment to review the options available. The resident trialled several options to operate a computer such as grid pad, eye gaze and head pointing. The latter enables the user to access a computer using their head and eyes. 


The team determined that the grid pad technology using head pointing was the best option for the resident to access a computer. They felt it was less fatiguing to allow for the days ahead at college. This technology had various other benefits. It could embed the ability to control the lights, blinds and television — all from a central tablet! 

Other Elements to Consider 

There were other elements of occupational therapy intervention to consider. The main points included the social and emotional aspects of re-integrating into education. 


The resident has experienced anxiety around their changes in function. They also worried about its perception in social situations. On admission to our Kingston Rehabilitation Centre, the resident did not want to access the community due to their anxiety.  


Working together, occupational therapy and psychology completed graded community access sessions and behavioural experiments. The main goal was to change the resident’s perceived experiences of going out into the community from negative to positive ones. 

The Outcome of Intervention

Following the intervention, the resident is now going out with the care team and family and friends to eat, alongside taking trips shopping and to the cinema.


During the team’s vocational rehabilitation intervention, they continued their joint working approach. The psychology team provided weekly support for anxiety management and adjustment therapy. This has supported a smooth transition into education.  

 

Overall, this has been a successful vocational rehabilitation intervention. Spanning months, the ultimate goal was to place the resident’s goals at the centre of the intervention. 


Speaking of the outcome, Leanne said: “I believe this case demonstrates the diversity of occupational therapy and the impact it can have. The resident has successfully started their course, and the assistive technology assessment has given access to education. This has provided control over their environment and independence.”
