Your loved one may need the support of a carer, but want to remain in their own home. We have a range of options to provide care for different needs, ensuring your loved one is safe, happy and comfortable in their home.
This month, we’re exploring what a home care visit involves, and how it can benefit your loved one:
What is Home Care? | About Our Care at Home Services
Professional home care often involves your loved one living in their own home, whilst receiving external support to continue doing so. It provides peace of mind for elderly people who’re living at home, and also for their families.
Here at CHD, we have four Care at Home branches across Hampshire and Surrey. Our home care visits can range from 30 minutes to full-time, live-in care, depending on your loved one’s needs. We can adapt the arranged care plan accordingly, if and when their needs change. We also provide transparent care for people with learning disabilities, physical disabilities and other special needs.
Our home care visits can include weekly grocery shopping, assisting with medication, helping with washing and self-care or enabling your loved one to engage in local community activities.
We have a range of home care packages that we can tailor to support your loved one’s needs and help them reach their personal goals.
What is Home Care? | Our Wellbeing & Let’s Get Social Package
Our Wellbeing & Let’s Get Social package suits those who need minimal support. The package provides peace of mind for your loved one and your family and ensures that they don’t feel isolated.
Our Care at Home team will make sure that they’re physically, socially and emotionally well. They'll ensure that they have access to the community as well as plenty of companionship and support.
These visits often include - but are not limited to - grocery shopping, a trip to the salon and day trips, or enabling the individual to engage in community activities.
Our Wellbeing & Reablement Package
We now offer support for those who’ve been in hospital. We can arrange this at short notice, for emergencies or scheduled - such as a planned operation.
We can ensure your loved one has a safe, prompt and comfortable return home. We’ll liaise with you, your loved one and other healthcare professionals such as the discharge team, occupational therapists and doctors to ensure continuity of care and a smooth transition back home.
This package is usually implemented for six weeks but can be flexible if your loved one needs support for longer. We can also include rehabilitation care if needed.
Our teams will assist with meal preparation and personal care. They can also arrange transport to follow-up appointments.
What is Home Care? | Our Living Your Best Life Package
Our Living Your Best Life package aims to provide regular support for your loved one. This package enables them to live as independently as possible.
Our Care at Home team will assist with your loved one’s daily routine and self-care tasks. They'll also help with light domestic chores, and provide opportunities for improving their mental and emotional wellbeing.
Our carers will arrange for them to get involved in local community activities and engage in their hobbies or interests. They’ll help to meet your loved one’s emotional and physical needs and ensure they have plenty of social interactions.
What is Home Care? | Our Dementia Service Package
If your loved one is living with dementia, you may find it difficult to focus on other aspects of your life.
Our Dementia Service package is a beneficial solution in these circumstances. Your loved one can retain a sense of independence and be able to remain in their home, with the full support of our carers.
We offer early support and intervention, and will help to maintain your loved one’s care needs to avoid hospital or long-term admissions. We can adapt the care plan as their dementia progresses to ensure your loved one is always receiving the most up-to-date care for their safety and wellbeing.
Our Care at Home team will ensure your loved one’s nutritional and medicinal needs are being met and support them with their day-to-day domestic activities.
We’ll also enable your loved one to engage in social groups and events to help them retain their social skills and improve their wellbeing.
Our aim is to provide an exceptional level of care, tailored to each individual, to enable them to live their best life.
