This is an incredibly difficult time for you and your loved ones, and you may be feeling overwhelmed.


Here at CHD, our priority is to ensure that we offer you as much support as possible. From helping you understand the process to offering emotional and practical guidance, we’re with you every step of the way.

 

We Treat Your Loved One With Dignity and Respect

When your loved one requires end-of-life palliative care, we’ll ensure that every step of their journey is as comfortable and dignified as possible.


You can be assured that we’ll continue our high level standard of care at each stage; we’ll keep your loved one clean and comfortable and their space clutter-free and homely.


If your loved one likes to have manicured nails or wear some makeup, we’ll do this for them. Little things like looking their best can help your loved one feel much more relaxed during end-of-life care.

End-of-Life Palliative Care: We Deliver Person-Centred Care

Our person-centred care focuses on allowing your loved one to live freely. We accept and enable them to have the freedom to be who they are; whether that’s their religion, beliefs, ideas or culture, we’ll always respect their traditions.


It’s also about supporting your loved one to retain their independence. We’ll seek your contributions when creating their care plan and involve you with any decision-making, to ensure you and they are comfortable with their end-of-life care.

End-of-Life Wishes

Many people have hopes and dreams that they want to fulfil before the end of their life. Here at CHD, we help many of our residents achieve those wishes when they move into our care homes. From ice skating to getting tattoos, we’ve always gone above and beyond to help residents live their best lives.


When your loved one then requires end-of-life palliative care, we will endeavour to help them realise their dreams.


This will not only help them to live their best possible life, but it’ll also reassure you that they’re comfortable and cared for. This will help you to feel more at ease during their end-of-life care, and help to ease the difficulties you’re facing as a family.


We’ll seek your advice on what your loved one would enjoy, and help you to be a part of making some incredible, lasting memories with them.

End-of-Life Palliative Care: Emotional & Practical Support

There are many factors to consider when your loved one requires end-of-life palliative care. We will help to relieve the stress where possible by ensuring that you have enough support to deal with the practicalities. This can be from helping you understand the funeral process to arranging their personal requests.


We can also offer you emotional support; whether this is a shoulder to cry on or bringing cups of tea for the family. We understand what you’re going through, and will help in any way possible.


Please contact a member of our team for more information about the support we can offer you during your loved one’s transition to end-of-life palliative care. However, we would always recommend that you seek advice from your GP for bereavement services or grief counselling if you’re feeling low.
