Welcome to CHD Complex Care, part of CHD Living’s award-winning group of care services.

We at CHD are proud to provide industry-leading, specialist and high-quality personalised homecare for people with complex neurological conditions and respiratory conditions. We provide a person-centred package delivered by our highly trained and skilled carers.

All of our complex home care packages are supervised by nurses, with advice from consultants and clinical experts who collectively have decades of experience in leading neurodisability services.

Mission

‘Listen’ and Provide support to our clients through a specialised MDT with equal focus on improving independence, improving clinical status, and improving quality of life. Provide support to our care team¹ through specialist clinicians² who work alongside nationally and internationally renowned experts³.

Our staff are equipped to manage people with respiratory conditions, ventilator support, or people dependent on tracheostomy.
A ‘specialists-led care team’ provides the best respiratory care, giving clients the stability and assurance that they are in safe hands. Respiratory interventions include:

  • Tracheostomy management
  • Tracheostomy weaning
  • Non-invasive ventilation
  • Invasive ventilation
  • Weaning from ventilation

Conditions managed:

  • Spinal cord injuries, Neuromuscular disorders (Motor Neurone Disease), critical illness neuropathies, Muscular Dystrophies, severe brain injuries/Disorders of consciousness, Palliative care

We work with Respiratory Consultants, Consultant Respiratory Nurses from nationally renowned ventilation outreach units to enhance our client’s care.
Our care staff are supported by specialist clinicians (Consultant in Rehabilitation Medicine, Specialist Nurses, Physiotherapists, Occupational therapists, Speech and Language therapists, Psychologists) who act as our partners.
Our panel of nationally and internally renowned experts are available to advice the specialist clinicians and provide second opinion where needed.

Brain injury care

We have the expertise to care for a variety of people who will be transitioning from one clinical setting to another or from one stage of their rehabilitation to another.

Our care team are trained to work with multidisciplinary professionals who will be assisting clients with their rehabilitation or long-term care/ management.

Transitioning could be from a paediatric setting to an adult setting.

Spinal Injury Care
Our expertly trained team of carers are able to support people living with spinal cord injuries to remain in their own home. Our care team is led by a 'team lead' who will be an experienced carer and will guide the team on a day-to-day basis. 

Specialist clinicians are on hand to provide advice to carers on clinical management and work alongside Case Managers to achieve set goals. 

A risk-based care plan will be formulated by the clinical team with clear unambiguous instructions to the care team on the rehabilitation and management plan. 

Our multidisciplinary team train the carers in key areas including managing autonomic dysreflexia, management of catheters, use of assistive technology devices to promote independence, administration of medication, non-invasive ventilation, the management of PEGs/RIGs and tracheostomy care. 

Where patients need additional clinical advice, we have access to nationally and internationally renowned experts to provide these services. 

We also have partner specialist service providers who can complete 24-hr postural management assessments, wheelchair and bed positioning system assessments and prescription, spasticity management using Botulinum Toxin (Botox), splinting and signposting for additional specialist services.
Transitional care

Neurological care
Our carers are trained by specialist clinicians with extensive knowledge of neurological conditions.

Our team are equipped to provide care for people with a variety of neurological conditions including Stroke, Traumatic Brain Injury, Hypoxic brain injury, Spinal Cord injury, Multiple Sclerosis, Parkinson’s Disease, Huntington's disease, Guillain-Barre Syndrome, Disorder of Consciousness, Muscular Dystrophy, Cerebral Palsy.

Our teamwork with Multidisciplinary team and carry over tasks that assist with rehabilitation, maintaining stability and preventing complications from arising.

Care Team
Many of our care team have years of experience in working in the health care sector. All of our carers go through a robust interview process followed by a period of induction and shadow training to help them embed into their role and continue to receive regular supervision from a team leader a registered clinician.

We are lucky to have some carers having joined us from overseas whom hold nursing or other professional qualifications from their country of origin and bring valuable experience in respiratory care (tracheostomy/ventilators), post-surgical care, neurorehabilitation and A&E experience.

Individual care teams are developed based on each client’s specific needs.
Registered Nurses

The Registered Nurses working within our team have extensive experience in specialist areas including:

  • Management of clients with a tracheostomy and those dependent on ventilators.
  • Care of Percutaneous Endoscopic Gastrostomy (PEG), Radiologically Inserted Percutaneous Gastrotomy (RIG) and Jejunostomies

Our Registered Nurses provide support to the care team with training and competency assessments to provide them with the skills and understanding to deliver tracheostomy care.
They also provide on-call support to the care team when managing patients with neurological and respiratory issues, and impart knowledge within the care team about critical clinical issues including:

  • Management of epilepsy
  • Ventriculo-peritonial shunt management
  • Colostomy care
  • Non-invasive ventilation
  • Catheter management
  • Medication administration
Allied Health Professionals
We strongly believe a holistic team approach is essential to provide all care and rehabilitation interventions for the patients effectively.

We work closely with our partner AHPs (Consultant in Rehabilitation Medicine, Physiotherapists, Occupational Therapists, Neuropsychologists, Speech and Language Therapists, Dieticians) and Case Managers. These partnerships create a well-rounded team who work together to benefit the client, with the client truly at the centre.

Our partner AHPs have over eighty years of collective experience with some in centres of excellence, and each possess extended scope skills such as respiratory management, injection therapy, assistive technology, 24-hour postural management and ultrasound guided injections.

Our partner AHPs act as advisors to our care team and can signpost them, and Case Managers where required, to external agencies for specialist input.
Experts
The healthcare experts we associate with hold professorial and Consultant positioning in universities or leading Hospitals.

Advice is sought from the experts in unique situations where a second opinion or specialist interventions are required.

These experts have national or international reputation in their clinical areas and have contributed to the scientific literature in the form of publications and contributing to national guidelines.

We work closely with Case Managers and Brain injury lawyers who have engaged us to establish a care package for clients with complex needs as well as with Continuing Healthcare teams within the Integrated Care Boards in the NHS.

Patients or their families can also directly approach us to discuss their clinical needs and to arrange an assessment to set up a care package.
