Welcome to CHD Care at Home, part of CHD Living’s award-winning group of care services.

As physical and mental health needs become more demanding, it’s common for the individual and their families to experience heightened stress and anxiety.

We created our domiciliary care offering to help give some peace of mind to the elderly living at home and their families. From visits once or twice a day to 24/7 at-home support, our care team is dedicated to providing tailored services to those who need it most.



With 4 branches across Surrey and Hampshire, our multidisciplinary teams are dedicated to transforming lives for the better within their local communities. We provide home care to people of all ages, including those living with learning disabilities, physical disabilities and other special needs.

Our care packages range from thirty-minute visits at any time of day, right through to full-time, live-in care. Our personalised care and support packages are based on your individual’s wishes, desires and goals. This transparent approach to care, combined with clear, on-going communication is what allows us to cater to a wide range of conditions.

Daily Visiting
We provide a daily visiting service between the hours of 7am to 10pm, throughout the year.

With the involvement of your family / carers, our assessors will assist you in creating a person-centred care package to best suit your needs.
Range of Bespoke Care Services
Our care packages range from 30-minute visits to full-time, live-in support and companionship.

We can provide an array of bespoke home care services, tailored to your wishes, desires and goals.
Out-of-Hours Support
We operate an out-of-hours call service ensuring someone is always on-hand to support you.
Post Hospital Discharge Care & Support
At CHD Care at Home, we can provide a range of hospital discharge care options at short notice - whether you require support on an emergency basis, or following a scheduled hospital visit (e.g. planned operations, such as hip replacement or major surgery).

Our specialist home care management team will work closely with you, your family and other healthcare professionals to ensure you’re able to enjoy a safe and prompt return to the comfort of your own home. Throughout your journey, you’ll often work alongside various members of our healthcare team to determine the best-personalised care plan for you. This may include the discharge team, occupational therapists, social services, doctors and district nurses.
Personal Care & Medication Management
    Personal care, including:
    • Washing/bathing/showering;
    • Dressing;
    • Shaving;
    • Management of incontinence care;
    • Support with mobility and mobility equipment;
    • Support with prescribed medication

Practical support, including:

  • Meal preparation;
  • Light housework;
  • Laundry;
  • Shopping and collection of medication;
  • Sit-in and companionship*

*Additional transport and/or chaperon services may be available by prior arrangement.

Live-in Care
  • Our caregivers can respond to an increase in dependency levels by offering live-in support. Care recipients are able to remain in the comfort of their familiar surroundings, enjoying greater flexibility and personalisation.
  • Live-in services can be provided on a respite or long-term basis, and caters to those aged 18 or over with a range of dependency levels.
On-call Care
  • Care and support can be arranged within 48-72 hours, to support emergencies on a short-term basis. This includes convalescence support following hospital discharge.
  • We offer a sleep-in/waking night service.
Respite Care
Our respite service ensures continuity of care in your own home, whilst giving existing carers or family members a welcome break. Alternatively, you can check-in and enjoy a respite period in one of our CHD Living Wellbeing Centres.
Stay Connected
  • Use our Birdie app to stay up-to-date with your loved one’s care - on a real-time basis.
  • Enjoy peace of mind with instant access to an overview of each visit, live medication reports, care logs and wellbeing reports.
